چکیده

Abstract Aims Heart failure (HF) remains a major therapeutic and diagnostic challenge. In this study we aimed to analyze the predictive potential of four novel cardiovascular biomarkers – soluble urokinase-type plasminogen activator receptor (suPAR), vascular cell adhesion molecule (CAM-1), heart-type fatty acid binding Protein (H-FABP) growth/differentiation factor 15 (GDF-15) in detecting cardiac decompensation patients with HF. Methods total, 136 (40 decompensated HF, 96 compesated HF) were retrospectively enrolled serum biomarker concentrations GDF-15, suPAR, H-FABP CAM-1 analyzed by Enzyme Linked Immunosorbent Assay (ELISA). Biomarker correlated clinical biochemical parameters patients, value for was assessed ROC-plots, as well univariate multivariate logistic regression analysis. Cut-offs calculated means Youden index. Results A significant increase levels suPAR (1.6-fold change, p<0.0001), HFABP (2.2-fold p=0.0458) GDF-15 (1.7-fold p=0.0009) detected all HF compared compensated analysis, plasma H-FABP, significantly associated risk decompensation. For CAM-1, association remained even after correction confounders Additionally, AUCs (GDF-15: 0.759, 95% (0.683–0.866), suPAR: 0.838, (0.750–0.926), CAM-1: (0.750–0.926)) optimal cut-offs 1404.8835 pg/ml, 4774.027 4.74027 ng/ml) calculated. Correlation analyses found such creatinine inflammatory marker CRP be (r>0.3, p<0.001. conjunction results AUC NTproBNP (AUC=0.785), (AUC=0.838) (AUC=0.838 more accurately prediced heart than (Figure 1). Conclusion conclusion, investigated could valuable tool facilitate decisions suspicion Parameters renal function should taken into account. Funding Acknowledgement Type funding sources: None.

Predictors of low cardiac output in decompensated severe heart failure

OBJECTIVE To identify predictors of low cardiac output and mortality in decompensated heart failure. INTRODUCTION Introduction: Patients with decompensated heart failure have a high mortality rate, especially those patients with low cardiac output. However, this clinical presentation is uncommon, and its management is controversial. METHODS We studied a cohort of 452 patients hospitalized w...

Current Diagnosis of Patients with Acute Decompensated Heart Failure

Heart Failure Etiology The potential etiologies of ADHF are multifactorial. They can be broadly divided into two major categories: (1) the underlying etiology of the heart failure (the baseline cause of dysfunction), and (2) the etiology of the acute precipitant that resulted in worsening from the chronic compensated state. For some patients, particularly those presenting for the first time, th...

Heart Failure: Biomarkers of Diagnosis and Prognosis

The natriuretic peptides (NP), including the precursor N-terminal proBNP (NT-pro-BNP) of biologically active B-type natriuretic peptide (BNP) represent the current standard for identifying Heart Failure. In addition, the troponins serve to help identify Heart Failure patients at high risk for adverse outcomes. For nearly the last decade, emergency physicians have used these biomarkers to diagno...

Decompensated heart failure

Heart failure is a disease with high incidence and prevalence in the population. The costs with hospitalization for decompensated heart failure reach approximately 60% of the total cost with heart failure treatment, and mortality during hospitalization varies according to the studied population, and could achieve values of 10%. In patients with decompensated heart failure, history and physical ...
